Let's check whether your life capability is depressed in order to live active life. Fill in the life capability evaluation "Basic Check List" sent in the middle of April and send it back to Senior Citizens Care Section. To the person who has possibility of capability depression in the result of the entry, we send a medical consultation card to check up in the medical institution.

• Target: Person in the age of 65 or more except the certificated of support
  and long-time care need
• Consultation Fee: Free

The national pension premium of the year 2010 is 15,100 yen per month. There is a discount of 50 yen per month for the debit in the end of the month by the account transfer (early discount). Or when you pay in advance the amount of a year at a banking facility or a convenience store, there is a discount of 3,220 yen.

It is a complete medical checkup targeting the person who joins Takaoka City National Health Insurance. Apply directly at the designated institution.

• Target: Person in the age of 30 - 70 (40-70 years old for the brain checkup) who join the national health insurance and who is not behind the payment
• Checkup Period: April - December
• How to Apply: Make a reservation at the designated medical institution you desire, telling "the checkup by Takaoka City National Health Insurance, fill in the application form, and hand it in to the window of Health Insurance & Pension Plan Section. Notice of qualification will be handed.
• Place where there is Application Form: Health Insurance & Pension Plan Section, Designated Institutions
• How to Receive Checkup: Bring the national health insurance certificate, notice of certification for complete medical checkup and self-paid money with you to the designated institution on the day of checkup.
  ・Aid of the city can't be received if you don't have the notice of qualification for complete medical checkup. And the aid is once a year.
  ・As for the details of the checkup, confirm at the designated institution.
  ・The result of the checkup is notified from the institution to not only the recipient but also to the city in order to make use of for the health concern.
  ・Information is carried on the website of Health Insurance & Pension Plan Section （http://www.city.takaoka.toyama.jp/fukushi/0704/index.html） and "Health Guide" distributed at the same time as Citizens and Municipal Administration April Issue.

The person whose payment of insurance premium is withdrawn from the pension is continuously "the payment withdrawn from the pension" after April too.
And the person who received "the beginning notice of special tax collection" in February is changed your payment from "by the payment slip or the account transfer" to "withdrawn from the pension" after April (The person who notified the change of the way of payment is excluded.)

A part of taxi fare is aided in order that the heavily disabled person can go out easily.

  1. Person in the first or second grade of the function disorder of both legs or body trunk
  2. Person in the first grade of impaired vision

Physical Disability Certificate by liver function disorder is issued from April.

• Target: Person who has liver function disorder corresponding to the qualification standard, Person who has received the liver transfer and takes anti-immune therapy
