  ・Copy of the bankbook of the applicant (guardian) (which can show the name of banking establishment and the branch, account number and account holder's name)

Requirement of the aid is that the child joins the national health insurance and the guardian's income is within income limitation conformed to child-support allowance. The aid is for the self-pay burden and excludes the meal and medical expense on admission and the cost outside of the health insurance treatment.
