When one month medical cost of the person who has joined in National Health Insurance becomes expensive, the excess of the self-pay burden maximum is reimbursed by application. However the person more than 70 years old doesn't need to apply.

◆Calculation of the self-pay burden maximum

○Household on which residential tax is imposed

• Household of upper level income…150,000 yen ＋ (medical care cost − 500,000 yen) × 1% (after 4 times 83,400 yen)
• Household of general income…80,100 yen ＋ (medical care cost − 267,000 yen) × 1% (after 4 times 44,400 yen)

○Household on which residential tax is not imposed

• 35,400 yen (after 4 times 24,600 yen)
  ※Household in which total income after basic deduction exceeds 600,000 yen, or there is a family member whose income can't be confirmed

• Things to bring: National Health Insurance Certificate, Receipts of medical institutions, What to recognize your account number for transfer
• Place of application: Health Insurance & Pension Plan Section, Citizen's Affairs Section (Fukuoka Office), Fushiki, Toide and Nakada Branch

◎If you are late to notify of joining

Qualification of National Health Insurance isn't on the day when you notified to the City Hall but the next day when you moved in or retired from the business place (the day of the fact) and National Health Insurance Tax is calculated dating back to that day.

◎If you don't notify of withdrawal

You are about to pay both National Health Insurance Tax and another health insurance premium.

◎In the case of birth

Lump-sum payment for maternity and nursing benefit is directly paid to the medical institution by the direct payment system from October, 2009. (The bill from the medical institution and so on is less than 420000 yen, application procedure for the difference amount is needed. Please bring your National Health Insurance Certificate, Maternity & Child Health Handbook, Bank passbook, Detailed statement of expenses and Consensus document.)

◎In the case of death

Funeral cost is paid through account transfer. Please bring what to recognize the name of your banking organ, account name and number.

◎Retired Person Medical Care System

Person under 65 years old who had joined the workplace's pension system for more than 20 years (or 10 years for over 40 years old), and now receives pension and his or her dependant take medical treatment with the Retired Person Medical Care System. Please bring your annuity certificate of such as employee pension.

○When you join National Health Insurance

• When you moved in from another municipality
  What to need: Certificate of moving out

• When you quit the health insurance of your workplace
  What to need: Certificate that the health insurance of the workplace expired (Qualification loss certificate)

• When you are no more a dependant of the workplace's health insurance
  What to need: Certificate that dependency of the workplace's health insurance expired (Qualification loss certificate)

• When you quit arbitrary continuous insurance on the way
  What to need: Certification of arbitrary continuous insurance, Payment notice (still non-paying)

• When a child was born
  What to need: National Health Insurance, Maternity and Child Health Handbook
  ※Please bring your annuity certificate when you correspond to the Retired Person Medical Care System, National Health Insurance Certificate of your family member when you join additionally.

○When you quit National Health Insurance

• When you moved out to another municipality
  What to need: National Health Insurance Certificate

• When you joined health insurance of your workplace, or when you became a dependant of health insurance of the workplace
  What to need: National Health Insurance Certificate and Health Insurance Certificate of the workplace

• When you died
  What to need: National Health Insurance Certificate, Bank passbook

○Other cases

• When your address was changed in the city, when the head of household or family name was changed and when a household was divided or when plural households was made into one

• When you live in a distant place for school and so on
  What to need: National Health Insurance Certificate, School Certificate (Acceptance letter and Copy of student ID card are available.)
  ※Health Insurance Certificate for Student is issued.

• When you lost or made the Certificate dirty
  What to need: What you can prove your identity (Driver's license, Bank passbook, etc.)
  ※If you have Senior Citizen Recipient Certificate, bring it.
