The report on the pension account number is needed when you've changed your address.

• Category 1 insured person (self-employed, student, etc)…to the City's Health Insurance and Pension Plan Section when you've moved into Takaoka from another area. Report is not required when you've moved inside the City.
• Category 2 insured person (Employee's Pension Insurance or Mutual Aid Association), the category 3 insured person (Category 2 insured person's dependent spouse)…to Social Insurance Office in charge through your company
• Recipient of National Pension…to Social Insurance Office in charge

Person who has become 75 years old in May - August of this year will receive "The Latter Period Senior Citizen Physical Check-up" Consultation Ticket, Q & A Diagnosis Card, and List of medical institutions in the city designated to execute the physical examination at the end of September. Please inquire the date and hours at the medical institution you expect.
