◇The Approval of the National Health Insurance Certificate
When hospitalized as a National Health Insurance Member, window payment will become individual payment by showing the "Application Approval Certificates" to the medical institution. When it has been decided to go to the hospital, please issue your application.
This is targeted to the following people: Member of the National Health Insurance who is under the age of 70 and a person who is currently under insurance (who currently does not have any failures to pay the National). However, this can only be used when going to the hospital.

◇General Burden Reduction Certificate
When hospitalized as a National Health Insurance Member, your meal burden fee will be reduced by showing the "General Burden Reduction Certificate" to the medical institution.
This is targeted to the following people: A member of the National Health Insurance and who are exempted from paying city/ prefecture taxes.

◇The Dispatch of the National Health Insurance for the Elderly Application
Among the members of the National Health Insurance whom are 70 years and over, we will dispatch "National Health Insurance Certificates for the Elderly" towards the end of July (This does not including the members of the Medical-Care System for the Elderly). Since there is a partial burden reduction fee at the window of the medical institution, please show your health insurance card when you receive your medical examination/ treatment. The burden ratio (10% or 30%) of the identification of old recipient is fixed by a household unit based on income and the income in 2008 of the National Health Insurance household as by August 1. Furthermore, since the increase of the partial burden fee from the public's money (10%→20%) will be left unredeemed until March of 2010, the identification of an old recipient's expiration date will be March 31, 2010.

※The expiration date of the 10%burden is mentioned after the parenthesis that embeds the 20% burden mark.

You should have a special examination by Friday, July 31. Please arrive to get a check up with a Q&A card which lists out your consultation ticket, health insurance card, and any other necessary matters.

The consultation ticket is necessary for an medical examination.

We recommend using a fund transfer in order to prevent the forgetting to pay the national pension insurance fee. Once the documents are filed, it will take the burden of taking the time in paying the fee each month. Please apply for a fund transfer at the social insurance office, city hall, financial institution, or the post office. And do not forget to bring your pension book, bankbook, and your seal impression stamp.

○Notice About the Insurance Fee
We will mail you a letter with the senior citizen medical insurance fee for 2008 in mid July. Those, whom went through the special collection of taxes in April, will change from a temporary tax collection (income in 2006) to an actual tax collection (income in 2007). Furthermore, these fixes in tax payments will occur in October.

○The Update of the Health Insurance Card
The new health insurance card will be mailed at the end of July as a simple registered mail. The person receiving the mail must have the seal of the recipient or a signature if necessary. If the no one is at the house, we will leave a notice, so please contact the mailing company that is written on the letter. You may either ask them to redeliver the health insurance card, or you may pick it up at the post office. The new health insurance card is valid from August 1 to July 31, 2010.

○The Standard Income Amount Application
Part of the burden ratio (10% or 30%) from medical institutions, which are mentioned on the back of the health insurance card, is already decided through the municipal tax situation of 2009. If the standard taxation price in the bank book is more than \1,450,000, then the person himself and the family members who are insured will bear 30% of the burden ratio. However, if the income amount falls under the next list, then you will bear 10% of the burden ratio because of the application.

◆The Income of the Household
・If there is a person insured in a household.
If there is only one person insured in the household…Under \3,830,000.
If there is someone who is 70 years of age or older…The total money will be under \5,200,000.
・If there are two people or more in the household…The total money will be under \5,200,000
When you fall under, after checking the burden ratio that is written on the newly delivered health insurance card, please apply for a new one.

○The Application of the Price Limit/ Standard Burden Reduction Certificate
It is by law that one must apply to the "Application of the Price Limit" and the "Standard Burden Reduction Certificate". Every person in the household, who had a residence tax exemption in the year of 2009, will be able to be granted a price limit certificate. One will be able to lighten part of the burden from the hospitalization medical cost by showing the price limit certification at the window of the medical institution. Furthermore, part of the meal charges will also be reduced.
